How NHP works

You can reach out to NHP for general guidance or to be directed to helpful resources and this does not require enrolling in the program.

Step 1

Entering the Program

The program serves nurses across Ontario. Contact us for more information.

Step 2

Enrolment and assessment

Following your initial outreach to NHP, a case manager may get in touch to conduct a preliminary evaluation. This evaluation helps determine your eligibility for the program and identifies the types of treatment that may be appropriate. It also serves as the foundation for developing a personalized treatment and monitoring plan. Your health condition will remain confidential at all times. As long as you meet the program’s requirements, your participation will not be disclosed publicly.

Step 3

Treatment

NHP will create a personalized treatment plan to support your recovery journey. Throughout the program, your case manager will be there to guide you, offering ongoing support and helping you connect with an approved treatment provider if needed.

Step 4

Monitoring

Your case manager will collaborate with you to create a monitoring plan tailored to your treatment needs.

Step 5

Exiting the program

Once you have successfully completed your treatment and monitoring plan, you will be eligible to exit the program.

What is Expected From You

Once you begin with NHP, your base manager will explain what is expected of you as part of your care and follow-up plan.

Fees

Program-related costs may vary depending on your individual needs.
Your case manager will go over the specific costs with you and explain how they are covered.

Please note: You will not be charged for NHP case management services or for any independent medical examinations requested by the program.

Duration of Program

The duration of the program is determined based on your individual needs. It is informed by established best practices and evidence-based research that identify the appropriate length of treatment and support required for effective recovery from substance use and/or mental health disorders.

If challenges return

As a participant in the NHP, you will be assigned a dedicated case manager who serves as your primary point of contact. Your case manager will be available to address any questions or concerns you may have regarding your monitoring plan. This includes discussions related to adherence, challenges you may encounter, and the potential for relapse.

Your Privacy

No personal information will be accessed or shared without your explicit written consent, which will clearly outline the specific details to which you are agreeing. The NHP strictly adheres to the confidentiality standards set forth in the Personal Health Information Protection Act, 2004 (PHIPA). Our case managers are available to address any questions or concerns you may have regarding the collection, use, or disclosure of your personal and/or health information.
