The HealthPod is a self-contained medical clinic equipped with key biometric medical devices that connects users with a healthcare provider remotely to offer accessible and high-quality care and medical expertise only found in a hospital.

Why is the HealthPod Needed?
Many Ontarians struggle to access the care they need and face challenges navigating the healthcare system. In Ontario, 50% of patients wait 40 or more days to see a specialist after a referral from primary care. Additionally, 60% of patients who visit the emergency department could be managed differently with less intensive care if it were available.
The HealthPod has a small footprint and can be conveniently placed anywhere, such as office spaces, community centers or businesses which can reduce unnecessary down time related to addressing health concerns for individuals and the health system.
THP Solutions is looking for ways to tackle the challenges in navigating the system and getting the care in the right place at the right time. We are integrating research, innovation, and clinical expertise to develop, implement, test, and expand innovative solutions to reshape the way we access health care.

- Timely access to health care services at the time it is needed avoids unnecessary hospital visits and allows care to be provided closer to home or at work.
- Coordinated care is supported with referral to specialist consultations and diagnostic imaging with timely communication with primary care providers.

How does the HealthPod work?
What Services are offered in the HealthPod:
This HealthPod is equipped with key biometric tools to collect essential information to guide assessment and care. This includes medical grade Health Canada approved devices including to measure your blood pressure, heart rate, oxygen saturation, temperature, weight and video-enabled observation. Additional technological features are being explored for future enhancements.

What can I expect at the Virtual Visit?
You will be seen by a HealthCare Provider who will talk to you about your concerns. Depending on your needs, the following steps may be suggested:
- Sending a prescription to your pharmacy
- Making an appointment with a specialist
- Refer you for blood work or diagnostic imaging
- Follow up with your regular Primary Care Provider (PCP) or make a recommendation for follow up with other facilities taking new patients
What do I need for my virtual Visit?
You will need Your Ontario Health Insurance (OHIP) card, if you have one (you may still continue with your appointment if you do not have an OHIP card) or some other piece of identification.

The Bristol Circle HealthPod offers an opportunity for individuals over 18 years of age to conduct their medical appointment with a Nurse Practitioner or a General Internal Medicine Specialist virtually in the HealthPod:

Appointment Booking with the Nurse Practitioner (NP):
A Nurse Practitioner (NP) is a registered nurse (RN) with advanced education and clinical experience, enabling them to diagnose and treat illnesses, prescribe medications, and manage patient care independently. They offer a comprehensive range of healthcare services.

Appointment Booking with the General Internal Medicine Specialist (GIM):
General internal medicine focuses on the diagnosis and treatment of diseases in adults, with a particular emphasis on complex, multi-system conditions. Internists, as they are also called, are experts at managing chronic diseases, acute illnesses, and conditions that affect multiple organ systems.
1. To book an appointment you will need a referral from your primary care provider.

2. Once a referral is received you will be contacted for an appointment time.

Partners Community Health - Integrated Seniors Hub
The Integrated Seniors Hub at Partners Community Health is offering an opportunity for seniors 65 years of age and older living in Mississauga to conduct a medical appointment with General Internal Medicine Specialists virtually in the HealthPod.
General internal medicine focuses on the diagnosis and treatment of diseases in adults, with a particular emphasis on complex, multi-system conditions. Internists, as they are also called, are experts at managing chronic diseases, acute illnesses, and conditions that affect multiple organ systems.

- Clinic Hours: Appointments are booked between 9AM – 3PM. Mon – Fri.
- Appointments are available with a General Internal Medicine Specialist.
- All appointments with the specialist will require a referral from a physician or a nurse practitioner.
To book an appointment:

- Once registered, you can make an appointment with the Clinical Navigator at the Integrated Seniors Hub who can assess the need for further medical management. If needed they can recommend a referral to the Nurse Practitioner or back to the Primary Care Provider.
2. For those that have a Primary Care Provider (PCP):
a. You can download the referral form to share with your PCP add form
b. The Navigator can provide a paper referral form which can be shared with the PCP.
3. For those that do not have a Primary Care Provider (PCP):
a. An appointment with the nurse practitioner can be arranged by the Navigator in the Integrated Seniors Hub.
b. The Navigator can provide the referral form which can be shared with the NP.
c. The NP can send a referral to the Solutions Clinic.
4. Your PCP or NP can fax a referral to the Solutions Clinic at 416-521-4140.
